Important Things to Note

ADHD Assessments and Referrals:

  • Though Evolve does not currently offer ADHD assessments or referrals, attached here is a link to a website that will guide you through the “need to knows” and best options when trying to find somewhere that will support you in navigating an ADHD diagnosis.

Prescriptions:

  • 72 hours’ notice is required for all repeat prescriptions.

  • Prescriptions can be requested through My Indici, contacting reception via, email, text, phone call or by coming into Evolve and speaking to reception.

  • Urgent prescriptions will be considered but not guaranteed, as the legal time to process prescriptions in New Zealand is up to 72 hours (3 working days).

  • Evolve will contact you if more information is required to be able to prescribe your medication or if you need to book an appointment.

  • Evolve retains the right to decline to complete a request for a prescription, but we will advise you of this. This may be because you need to book an appointment for a medication review if you haven't been seen by an Evolve clinician in over six months.

  • You will be advised when your script is ready to pick up by a text message sent from the pharmacy.

Requesting Medical Records for yourself

  • If you are wanting a request of medical records for yourself rather than a medical centre (this can be the case if you are moving abroad). Come in to Evolve with a form of I.D and we can start the process then and there!

  • If you are no longer in Wellington, send us an email at reception@evolveyouth.org.nz and we will set up a zoom call to verify your Identity.

  • Your notes will be encrypted with a password of your choosing and are sent to you via email.

  • We will have your notes sent to you within 20 working days.

  • We have a zero-tolerance policy for aggressive behaviour. Anyone displaying this behaviour will be asked to leave immediately. Continued or serious incidents may result in termination of enrolment with Evolve Wellington Youth Service.
