Ageing Out of Our Services
Evolve is a free service for those aged 10-24. Once someone reaches 25 years old or moves away from Te Whanganui-a-Tara (Wellington), they are no longer eligible for our service.
To set you up to leave Evolve and enrol in a new GP practice, we have compiled some helpful information as you continue on your journey.
Registering for a new GP practice
To find out which GP practices in your area are open to enrol new patients check out Tu Ora’s map of GP practices in the area or the Healthpoint website:
Have a look around some different GP practices as consults can vary in price depending on the practice, click here to see a list of price comparisons (make sure to change the toggle from practices to practice fees). Each service will set their own price and have differences – however the overall structure will be the same. Check out where might be best for you and remember that you can change clinic as well as take along a support person.
We recommend starting this process early as there can sometimes be a wait to enrol with a new GP practice. We also recommend checking if the new GP has missed/ late appointment fees.

Financial support for accessing health services.
Disability Allowance
A weekly Work and Income (WINZ) payment of up to $82.85 to help pay for regular ongoing costs of a disability (including health conditions and mental health conditions) such as doctor visits, medication, and transport to appointments.
You do not have to be on a benefit to be able to get the disability allowance. Eligibility is based on your weekly income.
You will need to complete the application form, have a doctor complete the disability allowance medical certificate, and provide proof of costs (quotes or receipts) to apply for this.

Disability Allowance specific to Counselling
The disability allowance (maximum amount of $82.85 a week) can also be used towards the cost of counselling if your health condition or disability is likely to last at least 6 months and the counselling is related to your health condition/disability (e.g. depression, anxiety).
To use this for counselling there is a different form to complete which needs to be completed by you, your health practitioner and your counsellor.
If this is the first time you're applying for counselling, you can get up to 10 sessions. You can apply for more if a GP or nurse practitioner recommends them (however WINZ will not fund this long term)

Community Services Card
A community services card helps with the costs of health care and public transport. You can get reduced fees for doctor and nurse appointments, subsidised prescriptions, reduced fees for after hours doctor visits, public transport, emergency dental care and home help.
You do not have to be on a benefit to get the CSC. Eligibility is based on your income.

High Use Health Card
If you are someone who regularly visits the doctor (12+ per year), have a talk to them about getting the High Use Health Card which may reduce costs of future appointments.
