Enrollment & PHOs

Evolve is part of the Well Health Primary Health Organisation (Check out the Ministry of Health website for more details about PHOs)

If you are aged 17 or over and you are a New Zealand resident you are eligible to enrol with us as your main primary health care provider.
If you are 16 or under you can still enrol, but will need signed permission from your parent or caregiver.

What does enrollment mean for you?

• Evolve will become your main primary health care provider (like a family doctor)
• We will ask your permission to transfer your medical records to Evolve, where we will be responsible for keeping an overview of your health needs and records (these will be kept totally confidential)
• Our clinicians can get to know you better, so they will be able to offer the best possible support with any ongoing health needs.
• You can still use other health services – but you'll just need to register with them as a ‘casual’ patient
• Enrolling to a PHO could entitle you to extra benefits such as cheaper prescriptions & dental care
• You can un-enrol or change to a different service at any time and still use Evolve as a 'casual' patient
