Our Services

Evolve is a ‘one-stop-shop’ youth service which offers primary health care, sexual health, social support, counselling, peer support and activity based projects.

All of our services are free and confidential for New Zealand residents* aged 10-25.

How to 'Sign Up'

The first time you come to Evolve for medical services you will need to register as a patient.
Our receptionist will explain your options, which are to register as a casual or enrolled patient.
You will usually need to fill out a registration form, but if you would rather register verbally or need some help with the forms, we can either fill out the form for you or type your details straight in to the database.

For more information on enrollment and PHOs just ask at reception, or click here

How to get an appointment

If you are already registered with us you can make an appointment by calling or dropping in to reception.  It’s best to make appointments in advance where possible, as we don't always have appointments available on the day.

If you need to see a doctor urgently please try your regular family doctor, student health service, afterhours medical centre or a service listed in the 'medical centres' section at the front of your phonebook. (link)

In emergencies dial 111 or go to your local hospital emergency department.

*Non-residents

Evolve is able to see non-residents (includes visitors to NZ on work permits & student visas) aged 10-25 for a small fee:

Nurses (contraception & sexual health only)
 - $35 per consultation

Doctors (sorry no immigration medical exams)
 - $50 per consultation

Please note that you will be charged extra for any laboratory tests including blood tests, chlamydia swabs, cervical smears etc.

You must bring cash to your appointment and pay immediately, as we have no eftpos facilities.
