Nurse Practitioner (Youth Health)

I’ve been at Evolve since we opened in June 2004 and I since then I have completed my Masters in nursing and was accredited as a Nurse Practitioner in Youth Health. *
This means I can provide a wider range of assessment, treatment and health management options, including prescribing some medications.

I’m a friendly and approachable person, so you can talk to me about any worry or health concern and if I can’t help, I will find the right person who can.
In my spare time I like going to the gym, keeping fit, reading, hanging out with friends, having coffee and photography.
The thing I love most about Evolve is the young people!  And that everyone is different – so I never know who I’m going to meet and what they will need.  I also love the space we have and working in the city

Jan Jaggard

Primary Health Nurse

We are very pleased to welcome back Jan Jaggard, who has previously worked as a locum nurse at Evolve and has now joined our team for 3 days per week.  Jan is able to provide the full range of primary health nurse services, including sexual health clinics.  Check out our services page for more info on what nurses can offer.

Danielle Hann

Nurse - New Graduate


Danielle has joined our nursing team as a new graduate in primary health nursing and will be starting work at Evolve 4 days per week in 2011.  Danielle can provide all the basics of primary health nursing, but if you need something a bit more complex you'll need to make an appointment with our Nurse Practitioner or doctor.

Dorianne Page

Outreach & primary health nurse

Dorianne joined the Evolve clinical team in mid 2011 and provides a ranch of school outreach clinics, as well as health promotion.  She also provides some regular nurse clinics at Evolve.
